Eligibility<\/b><\/p>\n

Residency<\/span>: <\/span><\/b>DuPage County Residents<\/span><\/p>\n

Senior <\/span>(minimum age):<\/span><\/b> 60 under 200% of poverty<\/span><\/p>\n

Disabled<\/span>:<\/span><\/b> Any age under 200% poverty<\/span><\/p>\n

Low<\/span>-Income:<\/span><\/b> 200% of poverty for non seniors\/disabled<\/span><\/p>\n

General <\/span>Public:<\/span><\/b> Must be 200% of poverty for non seniors\/disabled<\/span><\/p>\n

Other <\/span>Eligibility Requirements:<\/span><\/b> If receiving Medicaid\/TANF cannot use program for medical trips<\/span><\/p>\n

Trip restrictions<\/b><\/p>\n

Medical <\/span>trips, DuPage County Health Department and DuPage County Community Services appointments only. Trips restricted to DuPage County Addresses only. Trips outside of the county must be pre-approved by the sponsor.<\/span><\/p>\n

$<\/span>4.00 each way to 421 and 111 N. County Farm Road, Wheaton. All other destinations are $1.50 for the first 6 miles and $1.50 each mile after that.<\/span><\/p>\n
